Medical Record, Documentation & Consultation Requests

Thomson Memory & Attention is introducing a new way to make requests for medical records, documentation, and follow up consultation sessions with your Thomson provider. Below is a description of each type of request in addition to a request form and our online payment portal for each request.
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal law that required the creation of national standards to protect sensitive patient health information from being disclosed without the patient's consent or knowledge. HIPAA states that your medical record will be retained for 6 years beyond the date of service. Medical records can be requested from Thomson that fall in this 6-year window.
Medical Records can be released to the following:
- A parent or guardian of a recipient who is under 12
- The recipient, if he or she is at least 12
- A parent or guardian, if the recipient is at least 12, but under 18, and is informed and does not object or the provider does not find compelling reasons for denying the access
- The guardian of a recipient who is 18 years or older
- Any entity or individual granted access by an executed ROI by the recipient
